Check Your Ayushman Eligibility & Download Your Ayushman Card Before You Need It

Quick take: Don’t wait until a hospital admission to discover whether your family is covered under Ayushman Bharat PM-JAY. Use the official NHA beneficiary system to check eligibility and, where eligible, complete the Ayushman Card process. Keep the card available on your phone and as a printout.

The official PM-JAY helpline is 14555. NHA materials also direct beneficiaries to PM-JAY digital services, CSCs and empanelled hospitals for eligibility and card assistance.

Why check before the emergency?

A hospital emergency is a terrible time to discover:

  • your family's beneficiary record cannot be found;
  • a family member's details need verification;
  • the Ayushman Card has not been generated;
  • you don't know which hospital is empanelled;
  • or your Aadhaar / beneficiary details need correction.

Step 1 — Check your PM-JAY eligibility

Use the official NHA beneficiary portal:

Depending on the beneficiary category and portal flow, you may be asked to authenticate using your mobile number and provide the identification details available to you.

If you cannot find your record, don't immediately assume you are ineligible.

Try the identifiers available through the official system or contact 14555 for assistance. NHA's beneficiary guidance has historically supported eligibility checking through its digital platforms, CSCs and the helpline.

Step 2 — If eligible, complete the Ayushman Card process

Once your beneficiary record is found:

  1. Follow the portal's authentication instructions.
  2. Complete e-KYC if the system requires it.
  3. Check that your name and other details are correct.
  4. Complete the card-generation process.
  5. Download/save the digital Ayushman Card if available.
  6. Keep a printed copy for elderly family members.

Don't pay someone simply because they claim they can "activate" an Ayushman Card faster.

Use the official portal, an authorised CSC, or the Ayushman assistance available through an empanelled hospital.

Important: If you are 70 or older

This is a major eligibility change many families still miss.

NHA states that all senior citizens aged 70 years and above are eligible for AB-PMJAY regardless of income. Aadhaar-based e-KYC is required for enrolment and issuance of the Ayushman Card for this category.

For this 70+ category:

  • Aadhaar is the required document for enrolment.
  • A distinct Ayushman Card is issued.
  • Treatment can begin from the first day of enrolment.
  • There is no waiting period for diseases or treatment under this senior-citizen expansion.

If both parents are 70+, check them individually rather than assuming that one parent's existing card automatically covers the other.

Having an ABHA does not by itself mean that you are eligible for PM-JAY.

Step 3 — Check the hospital separately

Having an Ayushman Card does not mean every hospital will provide PM-JAY treatment.

PM-JAY benefits are delivered through empanelled hospitals and are portable across India under the scheme framework. NHA's material states that beneficiaries can receive cashless treatment outside their home state at empanelled hospitals.

Before elective treatment:

Check the hospital → call the Ayushman desk → confirm the relevant package.

What to carry to the hospital

Keep a small health-document pouch containing:

  • Ayushman Card / digital copy
  • Aadhaar or other required identity document
  • mobile phone used for authentication
  • existing medical reports
  • prescriptions
  • previous discharge summaries
  • insurance documents, if applicable

For elderly parents, keep a printed card as well.

If the hospital says: “You're not covered”

Don't start with an argument.

Ask:

“Can you please check my beneficiary record and tell me the exact reason?”

Then note:

  • hospital name;
  • date and time;
  • Ayushman desk/person spoken to;
  • package or treatment requested;
  • exact reason for refusal;
  • any reference number provided.

If necessary, contact the official PM-JAY grievance/help channels through the scheme or 14555.

NHA's materials state that beneficiaries have rights within the PM-JAY system and that empanelled-hospital issues can be escalated through the relevant channels.

Don't confuse these three situations

“I am not showing as eligible.”

That's an eligibility / beneficiary-record issue.

“I am eligible but the hospital says this treatment isn't covered.”

That's a package / treatment coverage issue.

“The hospital is not in the PM-JAY network.”

That's an empanelment issue.

They require different conversations.

Don't wait until the morning of surgery.

Emergency

Don't delay emergency medical care while trying to solve a portal problem.

Take the patient's available identity and scheme documents and ask the hospital's Ayushman desk to check the beneficiary record.

Don't give your OTP to an “Ayushman agent”

An official service should not require you to hand your phone to a stranger.

Be especially cautious if someone says:

“₹500 do, card immediately activate kar deta hoon.”

Use the official NHA system or an authorised service point instead.

A small habit that can save a very bad night

For every elderly parent, keep three things together:

Aadhaar + Ayushman Card + important medical reports.

Don't wait until an ambulance is outside the house to search through WhatsApp.
